Works independently, according to established policies and procedures, to as! sist patients in coping with social and emotional problems that accompany an illness and hospitalization, and with difficulties in recovery and rehabilitation. In doing so, interviews patients and families to obtain psychosocial history, and assists patients in resolving environmental difficulties interfering with utilization of maximum benefits from medical care. In collaboration with other interdisciplinary health care team members, identifies and initiates a plan of care on patients in need. Prepares and updates case records including nature of social problems, patient and family progress, and treatment administered. Directs patients and families to appropriate community agencies or resources and follows up as necessary. Above services provided primarily to the Inpatient Rehabilitation Unit patients, but may also include any patients associated with Saint Francis Medical Center. Provides guidance and assistance to more complex cases. Works with patients aged neonate, ped! iatric, adolescent, adult, and geriatric.
Essentia! l Duties:
Interviews patients and family members to assess psychosocial needs including: level of function, cultural and ethnic factors, emotional status, social skills, family relationships, financial/insurance status, response to illness or current conditions affecting patient/family.
Completes an objective evaluation on each patient in the Inpatient Rehabilitation Unit and develops a comprehensive treatment plan, including long term and short term goals within the specified timeframe.
Involves patient and family members, as appropriate, in care plan development and process by meeting with them to discuss recommendations made by the healthcare team; attends interdisciplinary team meetings in order to provide social work input regarding the care plan process and psychosocial issues which may affect response to treatment, and to ensure a comprehensive and holistic approach to care provided.
Counsels patient and family members regarding iss! ues such as anxiety and stress precipitated by illness/hospitalization, coping with physical limitations/disability, fears related to helplessness or death, adjustment to the need for institutional or specialized care, etc.
Directs patients and families to appropriate community agencies or resources for financial assistance, counseling or support services. Provides ongoing facilitation and coordination of services in the community to ensure appropriate level of care.
Presents patient with discharge options so patient and family members can make informed choices. Conducts routine discharge planning, assessing the needs for home care or placement, and contacts nursing homes, community/state/federal resources on patient's behalf. May assist with making patient transfers by notifying facility, family of transfer, arranging transportation, processing documents/records.
Participates in or coordinates patient/family meetings and acts as patient advocate;! provides patient/family members with accurate information regarding pa! tient rights policy, advance directives, guardianship and power of attorney issues, etc.
Identifies and assesses possible victims of abuse or neglect, notifies proper legal authorities as required and facilitates interdisciplinary team meetings as needed.
Provides supervision and guidance to other social work staff as assigned for reviewing patients needs, case consultation, documentation, etc.
Records, documents, compiles and maintains monthly statistics on patients served and participates in various projects as assigned.
Attends/participates in departmental meetings/in-services and other interdepartmental team meetings (CQI teams) as assigned. Represents Social Work Department on various committees in-house or community organization meetings. Participates in education programs to the community as needed.
Accepts patients and families needing complex counseling or specialized casework services.
Participates in planning ! of social work services in selected hospital programs, i.e., outpatient and clinics, support groups, rehabilitation teams, etc.
Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature and attending related seminars, conferences and the like.
Qualifications
Education:
Master's Degree in social work with coursework in medical social work and gerontology required.
License/Certification:
Current Nebraska State Certification as a social worker required.
Must have and maintain at all times, a valid Nebraska driver's license and a good driving record. Must be able to utilize own transportation during work time.
