JOB TITLE: Medical Case Manager GENERAL DESCRIPTIONThe Medical Case Manager researches and recommends resources and creates flexible, cost-effective options for catastrophically or chronically ill or injured individua! ls on a case-by-case basis to facilitate quality individualized treatment goals, including timely return to work if appropriate. REQUIRED EDUCATION/TRAININGGraduate of accredited school of nursing. Current RN licensure in state of operation. Three or more years of recent clinical experience, preferably in rehabilitation. PREFERRED EDUCATION/TRAININGBSN desirable. Strong clinical background in orthopedics, neurology, or rehabilitation useful. Strong cost containment background, such as utilization review or managed care also useful. Certification as a CCM preferred. ESSENTIAL JOB DUTIES1. ! Provides medical case management to individuals through commun! ications with the patient, the physician, other health care providers, the employer, and the referral source. 2. Makes recommendations regarding health care resources.3. Develops Independent Medical Evaluation Plan. Provides assessment, planning, implementation, and evaluation of patient's progress. 4. Evaluates patient's treatment plan for appropriateness, medical necessity, and cost effectiveness. 5. Implements care such as negotiating the delivery of durable medical equipment and nursing services. Devises cost-effective strategies for medical care.6. Attends doctor's attorney's visits. Attends hospital and/or long-term facility discharge planning conferences, et cetera for the purpose of determining appropriateness of care and developing an effective long-term care strategy7. Assesses rehabilitation facilities for appropriateness.8. Performs or manages architectural assessment of patient's home. 9. Researches medical and community resources for patients with catastrophic! or chronic diagnoses, such as but not limited to, AIDS, cancer, spinal cord injury, diabetes, head injury, back injury, hand injury, burns, et cetera. Heavy travel required. 10. Minimum Billable Standards are 1.8 to 2.2 hours per case, per week, working with an average of 25 cases per month. 11. Requires regular and consistent attendance. 12. Comply with all safety rules and regulations during work hours in conjunction with the Injury and Illness Prevention Program (IIPP). 13. Additional duties as required. COMMUNICATION REQUIREMENTS Extensive clear and tactful communications required via writing, reading, telephone calls, note taking, letter writing, memoranda, et cetera. Must be able to negotiate.
