Location: Omaha, Nebraska

Primary Function Under the direction of the Director of the Community Mental Health Center (CMHC) or designee, the incumbent will be responsible for providing intensive case management services to clients in the mental health diversion program.

Primary Duties & Responsibilities:
Provide intensive case management services with clients including assistance with receiving appropriate mental health/substance abuse treatment, applying for benefits, establishing housing, and entering education or job placement. Consult with CMHC and other mental health professionals in order to develop an individual treatment plan and schedule appointments for appropriate mental health treatment. Work with each program participant to ensure they receive the necessary support services to achieve independent living. Plan, develop, and provide intensive case management. Work with families and the community to assist clients in planning for future needs and goals. Document and prepare necessary reports. Document activities in participant records according to CMHC policy and procedures. Gain basic information through the use of professional interviewing techniques. Maintain accurate records and charting. Transport individuals to various locations in the community. Develop and maintain working relationships with other agencies including the Corrections/Jail staff and officials, Boards of Mental Health, County Attorney and Public Defenders offices, City Prosecutor’s offices, Department of Social Services, Security, law enforcement, and other agencies. Represent CMHC in the community and in interactions with allied community organizations and service providers. Make professional decisions, recommendations, and maintain confidentiality. Recognize that individuals with co-occurring disorders are expected, not the exception, and are welcome into treatment. Assure services are provided in a co-occurring capable manner. Perform other related duties as assigned and/or as needed.

Qualifications:
Bachelor’s degree in Social Work, Criminal Justice, Psychology or directly related field of study. Two (2) years* experience performing case management in the mental health, human services or social services field. Experience working with the criminal justice system, and co-occurring populations preferred. Successful completion of a pre-employment criminal records check, including fingerprinting. Must possess and maintain a valid driver’s license and own mode of transportation at the time of hire and throughout the course of employment; must have and maintain a good driving record. *Education beyond the required Bachelor's degree may be substituted for the work experience requirement on a year-for-year basis.

Physical Requirements & Working Conditions:
The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Considerable time will be spent escorting patients to appointments and meetings at various locations outside a locked facility. Work may be stressful when dealing with individuals and/or working within time constraints. Work requires some physical activity including frequent sitting, standing, walking and reaching, and occasional bending, kneeling, crouching and stooping. An incumbent must have the ability to transport themselves to and from various locations throughout the Community Mental Health Center and other locations. An incumbent must have the ability to frequently lift and/or carry equipment, supplies and other materials weighing up to 10 pounds, and to occasionally lift and/or carry equipment, supplies and other materials weighing up to 50 pounds. An incumbent must have the physical ability to protect oneself and control combative or disruptive persons using measures specified within the CMHC psychiatric/behavioral emergency policy. An incumbent must also have the strength, agility and physical endurance to perform CPR. Work functions require that the incumbent possess sufficient hand-eye coordination and manual dexterity to use hands and arms to reach, handle, grasp and feel, and to operate computers and any other equipment that is used to perform the essential functions of the job. Required sensory abilities include vision, hearing and touch. Visual abilities, correctable to normal ranges, include close vision, distance vision, depth perception and the ability to adjust focus. Communication abilities include the ability to talk and hear within normal ranges.
