Essential Functions/Competencies

Conducts psychosocial assessments related to the type of illness/needs present:
Screens patients in assigned areas daily.
Establishes professional working relationships with staff

Collaborates with multidisciplinary team to understand the medical needs of the patients/families.
Appropriately documents assessment in chart.
Formulates patient goals on multidisciplinary/daily goal sheets.
Provides appropriate patient education and documents appropriately.

2. Provides referrals for financial and supportive needs.
Gathers information from patients/families.
Shares appropriate resource materials and information with patients/families.
Researches resources specific to patientâs medical condition.
Obtains necessary release of informa! tion authorization.
Provides follow-up as deemed appr! opriate.

3. Provides crisis/supportive counseling to patients/families.
Assists families with problem-solving and utilizing their positive coping strategies.
Provides interventions which reflect knowledge of crisis/counseling technique and is enhanced by disease specific knowledge.

Facilitates support groups as needed.

4. Provides assessment and coordination of child abuse/neglect cases.
Assesses psychosocial dynamics.
Communicates with medical and nursing personnel regarding assessment and plan.
Facilitates referral and provides ongoing communication with CPS, law enforcement, and county attorneyâs office.
Documents pertinent information and completes all reports in a timely fashion.
Prepares and presents cases at Childrenâs Advocacy Team. Att! ends on an as needed basis.

5. Provides on-call services for Social Work Department.

6. Covers for Social Work colleagues in their absence, and arranges for coverage of social work responsibilities when absent.

KNOWLEDGE, SKILLS AND ABILITIES:
Requires knowledge of human behavior, social problems, counseling modalities, and community resources.
Ability to communicate effectively both verbally and in written form, relaying case information to patients/families, physicians, and other healthcare professionals, attorneys and law enforcement as needed.
EDUCATION AND EXPERIENCE:
Masterâs Degree from an accredited college or university in Social Work required.
Minimum one year experience of supervised experience as a Social Worker in a clinical setting preferred.
CERTIFICATIONS/LICENSURE REQUIREMENTS:
Current Certified Social Worker or Provisional Masters Social Worker in the state of Nebraska required.
